It looks like he's handing him his ring, which he took off in the hospital, and after what it's done to him, you can't blame Alaric for being a little cautious about it. However, Damon has a point that he's better off wearing it, and Bonnie did perform that spell. Besides, it's better for them to have to find a way to help Alaric if he's alive than for him to die in this hunt. But will the spell hold if he puts the ring back on?
